Item9251: GenPDFAddOn header shifting is too simplistic

Normal New Extension GenPDFAddOn  
GenPDFAddOn simply shifts all headers of child topics in a hierarchy. The headers are shifted by the depth of the topic when using auto shifting.

This is too simplistic:
  • If a topic has multiple headers at the "top" level, then there is no entry in the TOC representing the child topic itself.
  • headers need to be normalized to the depth rather than simply shifted.
