Was entering a new task and it asked for me to login. When I got to the edit page, the form was missing. I attached it by hand. When entering this task where I did not have to login, the form was attached.

As a added usability issue, I expected the summary I had entered to appear on the edit page and was uncertain if I should proceed because all I saw was a "Title" entry box that was blank. My wiki users would find this very confusing as there are no instructions on the page or hints of what to enter such as in other bug systems. Having the subject or some (or all) of the form information visible might make this clearer for new or less technical users.
